On this page is a copy of our financial policy agreement. This agreement corresponds to the information provided when the appointment was made.
FOR PATIENTS WITH INSURANCE
An insurance information form is enclosed. Keep in mind that our records will only be as accurate as the information you provide to us. With today's technology, our claims are forwarded almost immediately after treatment is complete. If the information form is incomplete, we will be unable to submit a claim, and payment will be required at the time of service.
FOR PATIENTS WITHOUT INSURANCE
Payment for professional services are due at the time of the appointment. Should there be an extensive treatment plan, special arrangements can be made.
